        Electors of 16-Haflong(ST) LAC can submit the  following forms  duly filled   to District Election Officer, N.C.Hills, Haflong through email at deo-halflong@assam.nic.in or at his office at Haflong.

         Form 6- Application  for inclusion of Name in Electoral Roll

         Form 7- Application for objecting inclusion or seeking deletion of name in Electoral Roll

         Form 8- Application for correction of particulars entered in Electoral Roll

         Form 8A- Application for transpositon of entry in Electoral Roll
